Referral
Definition:
Interventions in
general education have been attempted and documented; interventions do
not alleviate educational difficulties and it is believed the student
may be disabled and in need of special education and/or related
services.
Referral Organization:
1. School-based: All school-based referrals shall originate with the schools student intervention team (I & RS)
2. Parent-based:
A. Parental referrals (must be in writing - email not accepted)
will not originate with the student intervention team; however, the
school counseling department will complete the appropriate district
referral packet prior to the timeline stated in the referral section of
this handbook.
B. The Early
Intervention Program (EIP) provides technical assistance to aid
transition. The EIP provides parents with a sample, formatted letter for
the parent-based referral.
3. Direct: The principal or designee shall provide documentation to the CST of the need for a direct referral. (Only to be utilized in a unique, urgent or crisis situation)
Reminder:
A complete
referral packet must be submitted for all referrals (school-based,
parent-based and direct). This packet must include the following:
- Detailed reason for the referral
- List of pre-referral interventions
- Completed referral form (signed by the building principal)
- Completed school nurse report
